Yes. Calyx offers telehealth appointments when appropriate.
Telehealth may include real-time phone or audio-video visits using a smartphone, tablet, or computer. Telehealth can be helpful for follow-up care, coordination, and non-emergency concerns that do not require an in-person visit.

Yes. Calyx offers Spanish-speaking support to help patients and families communicate clearly, ask questions, and feel more confident in their care.

Yes. Calyx provides in-home and facility-based visits for eligible adult patients.
Our providers may visit patients in private residences, family member homes, assisted living settings, nursing facilities, and other residential care settings. Calyx does not provide visits inside hospitals.

Calyx Practice Solutions partners with home health agencies, care teams, and referral partners throughout our service area to support patients who need accessible, coordinated medical care.
Most of our patients are referred through home health agencies. We are often contacted when an agency has a patient who does not currently have a primary care provider, or when the patient’s existing physician is not available to sign the orders needed for home health services.

A home health agency may refer a patient to Calyx when the patient needs physician support for home health services, including situations where the patient:
Does not have a primary care provider
Has a PCP who is unavailable or unable to sign home health orders
Needs medical evaluation to support ongoing home health services
Needs care coordination after hospitalization or a change in condition
May benefit from in-home medical visits or telehealth support
Calyx can evaluate the patient’s needs and determine how we can support their care.

Yes, when appropriate. If a patient is receiving home health services and does not have a primary care provider, Calyx may be able to serve as their PCP.
Once the patient is established with Calyx, our providers can help manage their care, coordinate with the home health agency, and support appropriate home health services.

If the patient already has an established PCP, Calyx can work alongside the existing provider when appropriate.
Our goal is not to disrupt the patient’s care team. Instead, we help coordinate services so the patient receives the right support, including skilled nursing, physical therapy, occupational therapy, speech therapy, wound care, and other medically appropriate services.

Calyx can support home health orders for established patients under our care when clinically appropriate.
If a patient does not have a PCP, or if their current provider is not available to sign required orders, the home health agency may refer the patient to Calyx for evaluation. After reviewing the patient’s needs and establishing care, Calyx can determine whether we are able to sign and manage the necessary orders.

Calyx can help coordinate medically appropriate home health services, including:
Skilled nursing
Physical therapy
Occupational therapy
Speech therapy
Wound care
Diagnostic services, when needed
Specialist referrals, when appropriate
We work with the home health agency, patient, family, and care partners to support continuity of care.

Yes, when clinically appropriate. If a patient needs diagnostic services, such as an X-ray or other testing, Calyx can evaluate the need and place orders as appropriate.
Some services may be performed by outside companies or mobile diagnostic providers.

Calyx evaluates each patient’s needs carefully. For certain medications, including Schedule II medications, Calyx may refer the patient to an appropriate specialist, such as pain management, for ongoing medication management.
This helps ensure the patient receives the right level of care from the right clinical provider.

Calyx serves Southern California:
Los Angeles, San Bernardino, Orange County,and San Diego counties.
If your agency serves patients outside these areas, please contact Calyx to confirm whether we can support the referral.

Calyx serves adult patients age 18 and older.
While many patients are older adults or individuals managing chronic conditions, Calyx’s services are available to eligible adult patients who need in-home, mobile, or coordinated medical support.

Calyx currently accepts Medicare Part B, Aetna, United Healthcare, and cash-pay patients.
For cash-pay patients, Calyx has a fee schedule available. Agencies may contact Calyx directly for current payment information and referral guidance.

In most cases, patients are referred to Calyx by home health agencies.
Calyx may accept a direct patient inquiry only when the patient is already receiving home health services and does not have a primary care provider or another physician available to sign the required home health orders.

Calyx aims to respond to referral inquiries as promptly as possible during business hours.
Response timing may depend on the completeness of the referral information, patient location, provider availability, and clinical need.

In-home and facility visits are typically scheduled 8a - 6p Monday through Friday, weekends if necessary.
Calyx provides non-urgent and non-emergency medical care. A provider may be reachable by phone or email for non-urgent matters, but urgent or emergency concerns should be directed to urgent care, the emergency room, or 911.

No. Calyx does not replace the home health agency’s clinical team.
Calyx provides medical provider support, care coordination, evaluation, referrals, and order oversight when appropriate. The home health agency remains responsible for delivering the authorized home health services within its scope.
